Katawa Shoujo
Katawa Shoujo is a visual novel released in January 2012 by Four Leaf Studios, an amateur international development team that famously formed on the 4chan imageboard. The project has an unusual origin story: it was inspired by a single concept sketch from Japanese doujin artist RAITA, and what began as an internet joke evolved over five years of volunteer development into a surprisingly sincere and well-regarded game. It was released completely free, which helped it spread widely and become one of the most well-known Western-made visual novels.
The story follows Hisao Nakai, a Japanese high school student who suffers a heart attack brought on by a congenital arrhythmia. After a long hospital stay, he transfers to Yamaku High School, a fictional school for students with disabilities. There he meets the game’s five main heroines, each living with a different condition: Emi, a cheerful runner with amputated legs; Hanako, a shy girl with severe burn scars; Lilly, a blind and gentle class representative; Rin, an armless painter with an unconventional way of seeing the world; and Shizune, the deaf student council president. The game branches into different routes depending on the player’s choices, with each path exploring Hisao’s relationship with one of the girls.
What made Katawa Shoujo notable was how it defied expectations. Given its provocative title (roughly “cripple girls” in Japanese) and its origins, many anticipated something crass or exploitative. Instead, the game earned praise for treating disability with unusual empathy and nuance, portraying its characters as full people rather than defining them by their conditions. Its central themes revolve around acceptance, adaptation, and moving forward after life changes irrevocably — as much for Hisao coming to terms with his heart condition as for anyone else. While it contains optional adult content, the writing focuses primarily on emotional connection and personal growth.
The game left a lasting legacy in the Western visual novel scene. It demonstrated that a free, fan-made project could achieve genuine emotional resonance and critical respect, and it inspired many players and developers to explore the medium. More than a decade later, it’s still frequently recommended as an entry point into visual novels, and it remains a touchstone example of an internet community project that turned out far better than anyone expected.
Main Character
- Hisao Nakai — The protagonist. An ordinary student whose life changes when a heart attack reveals he has a congenital heart arrhythmia. He transfers to Yamaku High School and must come to terms with his condition and his new life.
Main Heroines
- Emi Ibarazaki — An energetic, upbeat girl who lost both legs below the knee in a car accident. She’s the star of the school’s track team, running on prosthetic blades, and hides deeper grief beneath her cheerful exterior.
- Hanako Ikezawa — A painfully shy girl with severe burn scars covering half her body from a childhood fire. She spends most of her time reading alone in the library and struggles with social anxiety and trauma.
- Lilly Satou — A blind, elegant, and nurturing class representative of half-Scottish descent. Calm and motherly, she’s Hanako’s closest friend and carries her own family tensions beneath her composed demeanor.
- Rin Tezuka — An abstract painter born with tiny vestigial arms, who paints using her feet and mouth. Philosophical, blunt, and hard to read, her route deals with artistic identity and the difficulty of understanding others.
- Shizune Hakamichi — The deaf and mute student council president. Fiercely competitive, ambitious, and strong-willed, she communicates through sign language, usually interpreted by Misha.
Supporting Characters
- Shiina “Misha” Mikado — Shizune’s bubbly, loud best friend and sign language interpreter, recognizable by her pink drill hair. Almost always at Shizune’s side and central to her route.
- Kenji Setou — Hisao’s legally blind, paranoid hallmate who believes in a global feminist conspiracy. Serves as the game’s main comic relief and plays a memorable role in the infamous bad ending.
- Akira Satou — Lilly’s older sister, a laid-back lawyer who acts as a cool, supportive adult figure in Lilly’s and Hanako’s routes.
- Yuuko Shirakawa — The perpetually anxious, apologetic school librarian who also works part-time at the Shanghai café.
- Mutou — Hisao’s homeroom and science teacher, a bit awkward but well-meaning, who encourages Hisao’s interest in science.
- The Nurse — Yamaku’s friendly head nurse, who keeps an eye on Hisao’s heart condition and is a close friend of Emi’s.
