Operate Now: Knee Surgery is a medical simulation game where players take on the role of a surgeon and perform a full knee operation on a patient named Steve. The game walks you through every stage of a real procedure, from the first X-ray to the final bandage.

Steve arrives at the hospital after a skateboarding accident. His knee is in bad shape, and it’s your job to figure out exactly what happened and fix it. The diagnosis comes first. You position the X-ray camera over his knee, and the scan reveals a torn meniscus—the cartilage that cushions the joint.

Preparing the Patient

Surgery prep is a bigger part of this game than you might expect. Once Steve is in the operating room, you shave the area around his knee, disinfect the skin with a blue antiseptic liquid, and inject anesthetic so he won’t feel a thing. None of it is rushed. Each step matters, and skipping ahead isn’t an option.

The Surgery Itself

This is where the simulation gets interesting. You make small incisions with a scalpel, insert a drainage tube, and then feed in an arthroscope—a tiny camera that shows the inside of the knee on a monitor. Real surgeons use the same technique.

With the camera in place, you cut away the torn section of the meniscus using miniature scissors, remove the loose pieces with a grabbing tool, and smooth the remaining cartilage. Then everything comes out, and the repair work begins. You stitch the incisions closed, apply adhesive strips, and wrap the knee in a bandage. Done.

Two Difficulty Modes

New players start in Intern Mode, where an assistant named Lara explains every step as you go. It’s basically a guided tutorial, and it makes the whole procedure easy to follow even if you’ve never touched a surgery game before.

Specialist Mode is the real test. Lara’s hints disappear, and you have to remember the entire procedure on your own. The reward is double points—a fair trade for the added pressure.

Beyond the challenge, the game doubles as a light introduction to how arthroscopic surgery actually works. It won’t make anyone a doctor, but it gives players a surprisingly clear picture of what happens in an operating room.
